RPM-GDM

Thank you for using the RPM Gestational Diabetes Mellitus (RPM-GDM) app. This is for use by patients who have been diagnosed with Gestational Diabetes and receiving treatment within South Eastern Sydney Local Health District (SESLHD).

This app is designed to help you track your blood glucose levels (BGLS). It will also allow your care team to access and review your submissions when you are not in the clinic.

You should have already received instructions on how to enter this information into the app. You may receive notifications from time to time prompting you to enter information as required.

Our Team may occasionally contact you to see how you are going, ask for information, or arrange an appointment.

This app will also give you tips on how to manage your GDM and information on improving your health and wellbeing.
